The companies are joining forces in a three-year collaboration to target cardiometabolic conditions and maternal and child health in underserved communities. Leadership from Walmart and CareSource will discuss partnerships and scaling social determinants of health (SDoH) initiatives during a session at The RISE Summit on Social Determinants of Health on April 4 in Chicago.

Walmart and CareSource, a nonprofit managed care organization, have announced a partnership to address racial health disparities. The three-year partnership, which will start in Ohio and target cardiometabolic conditions, such as heart disease, stroke, diabetes, and hypertension, will use Walmart’s in-store community health workers (CHW) to gather health risk and social needs assessments and connect eligible CareSource members to community resources and education. Members will be connected to resources such as monthly grocery funds, a Walmart+ membership at no cost, telenutrition services, and access to CareSource Life®, including CareSource JobConnect™.

The partnership between the two companies will also include a maternal and child health program in Georgia, the state with the second-highest maternal mortality rate in the United States. Through the program, pregnant mothers or mothers who are 12-month post-partum and members of CareSource’s Georgia Medicaid managed care plan will have access to Walmart’s CHWs, monthly grocery funds, a Walmart+ membership at no cost, tele-nutrition services, and tele-doula services.
