The 5th annual RISE Population Health Summit will reunite mid- to senior-level professionals across population health management for unparalleled idea exchange, proven best practices, and emerging strategies to elevate population health efforts. Here is an inside look at six can’t-miss sessions RISE has planned:

A keynote address on reducing cardiovascular and kidney disease risks

The conference will kick off with a keynote address from Elizabeth Montgomery, national vice president, population health programs, National Kidney Foundation, on reducing cardiovascular and kidney disease risks through population health. She’ll discuss the barriers to member engagement and optimized experience for people living with kidney disease in a population health model; the population health strategies that health plans can employ to move beyond the Kidney Health Evaluation for Patients with Diabetes HEDIS® measure to ensure engagement and optimized health outcomes for their members with kidney disease; and ROI data that supports ongoing testing and early intervention for people living with kidney diseases.

A panel discussion on health care burnout

A group of industry leaders will discuss how value-based care and population health work impacts burnout amongst health care workers, the resources available to combat burnout to improve communication and building a support system, and how to get back in touch with what inspired you and the reasons you went into health care.

Panelists will include Jennifer Howard, MSN, RN, NE-BC, lead director, VBC performance engagement, AETNA; Dhruva Gulur, M.D., CEO and founder, Mind Hygiene™ Eutopia; and Becky Trotter, vice president of population health, CareAllies. Dr. Ellen Fink-Samnick DBH, MSW, LCSW, ACSW, CCM, CCTP, CRP, FCM, wholistic health equity strategist, content developer, educator, EFS Supervision Strategies, LLC, will moderate the discussion.

A talk on demonstrating strong ROI with the right resources

Jennifer Houlihan, vice president, value-based care and population health, Wake Forest Baptist Medical Center, and Janet Pogar, market leader SW Florida, Florida Blue, will present a session focused on data insights to maximize population health growth through ROI analysis, cultivating partnerships and engaging stakeholders to optimize resource allocation for population health, and strategies to showcase strong ROI and fuel population health growth.

An early risers’ Mind Hygiene™ meditation

Day two of the conference will offer an early morning meditation session for any attendees interested.

A focus on addressing health inequality

Fink-Samnick will return to the stage to moderate a discussion alongside Denise L. Peroune, Ph.D., executive director, population health, Nirvana Healthcare Management Services, and Cara English, DBH, MA, LAC, PMH-C, CEO, Cummings Graduate Institute, on strategies to activate equitable population health. The session will explore root causes of health inequalities in populations, current strategies to reduce health inequities in maternal health that empower community collaboration, and whole person approaches to healthy aging.

A session on generative AI in population health: Revolutionizing health care solutions and beyond

While discussing revolutionizing health care solutions and beyond, Hans Wiik, president & CEO, Hans Wiik Health Group LLC, and Chuck Marshburn, staff vice president of social data analytics and product development, Elevance Health, will talk about the pros and cons of generative AI in population health; the applications of generative AI in health care solutions, including predictive modeling and personalized medicine; and the future prospects and ethical considerations of generative AI in advancing population health and health care as a whole.
